FAB Professionals - Licensing Assistance

FUTURE FLORIDA SALON:

In order to accelerate the application review process for your salon license, please complete the enclosed application and mail the original documents to FAB Professionals for review. Please include the following items in the application packet:

Completed Salon Application for FAB Professionals and the membership fee appropriate to the anticipated number of chairs in your new salon (if you have not already paid online)
$105 check made payable to “DBPR”
Completed DBPR Application

· Form DBPR 0020 – Master Organization Application Form

· Form DBPR 4402 – Salon Licensure Application Form

It is highly recommended that you make copies of all portions of the application packet for your records before mailing and pay the additional fee with your mail carrier for delivery confirmation or some form of package tracking.

Our average processing time is 17-22 business days from the point of delivery to DBPR (once all required items have been received and reviewed). Although DBPR works very hard to process each FAB application within 30 business days, this is not a guarantee and our service is dependent upon functions within DBPR that are beyond our control.

Once verification is complete, FAB Professionals will hand-deliver the application to the Department of Business and Professional Regulation (DBPR) on applicant’s behalf and will follow its progress through the application process. FAB Professionals will notify applicant upon approval.

Please note: If applicant bypasses FAB Professionals and sends their application package directly to DBPR, FAB Professionals will be limited in its ability (and possibly unable) to assist members should they encounter any delays or issues with DBPR.
