FAB Professionals - Licensing Assistance

FUTURE FLORIDA LICENSEE:

In order to accelerate the application review process for your endorsement of your out-of-state license, please complete the enclosed application and mail the original documents to FAB Professionals for review. Please include the following items in the application packet:

Completed Individual Application for FAB Professionals and the $85 service fee (if you have not already paid online)
$80 check made payable to “FAB Professionals” (this enables us to pay your fee online through DBPR)
4-Hour HIV/AIDS Certificate - can be completed online here

· Form DBPR 0010 – Master Individual Application Form

· Form DBPR 4404 – Licensure by Endorsement Application Form

It is highly recommended that you make copies of all portions of the application packet for your records before mailing and pay the additional fee with your mail carrier for delivery confirmation or some form of package tracking.

Since FAB often utilizes DBPR's online processing portal, it is very common for applicants to receive "deficiency letters" requesting the HIV certificate. This is a result of the lag time between when the physical application gets married to the online application. Please do not be alarmed if you receive one of these letters. Rest assured, your complete application has been properly delivered to DBPR and is awaiting final approval.

Our average processing time is 17-22 business days from the point of delivery to DBPR (once all required items have been received and reviewed). Although DBPR works very hard to process each FAB application within 30 business days, this is not a guarantee and our service is dependent upon functions within DBPR that are beyond our control.

Once verification is complete, FAB Professionals will hand-deliver the application to the Department of Business and Professional Regulation (DBPR) on applicant’s behalf and will follow its progress through the application process. FAB Professionals will notify applicant upon approval.
